New Patient Resources
There are ten other dental offices when it comes to choosing a dentist near Long Island City, but we’re ecstatic that you’re choosing us. Our entire team welcomes you to the 1311 Jackson Ave Dental family.
As a new patient, you can expect a warm and friendly welcome from our dental team. We understand that visiting the dentist can be intimidating for some, which is why we take the time to get to know you one-on-one. Our goal is to not only learn your name but also your dental healthcare goals so that we can provide personalize care for you.

We thank you once again for choosing our dental practice as your partner in oral health. We look forward to serving you and your family for many eons to come. We will provide you with information about what you need for your first appointment as a new patient so you can experience the difference that our team can make for your dental health.
Your First Visit
During your first visit, we’ll give you a tour of our dental office and introduce you to your new dental team. You can consider this as a “meet and greet” with your new dentist who will review your medical and dental history.

What to expect during the consultation:
-
Take dental x-rays and intraoral photos of your teeth which will be displayed on a big TV in front of you.
-
Comprehensive oral examination to diagnose tooth decay and periodontal disease.
-
Professional teeth cleaning with our dental hygienist or our dentist.
-
Discuss necessary treatments and schedule future appointments.
New Patient Forms
You will check in with our dental receptionist and fill out all pertinent forms via a tablet. Yes, our entire office is in the cloud. We also have two way texting so you can text back to the appointment confirmations and a human at our office will respond back to you.

Note: Please be aware that we have a 48 hour appointment cancellation policy. We don't impose a monetary penalty for last minute cancellations but we may permanently dismiss patients if they do it twice in a single calendar year.
Accepted Dental Insurances
During new patient registration, it is important that we verify your dental insurance so that you know exactly how much coverage you'll have.

Please bring these to your appointment:
-
Dental insurance card
-
Subscriber's member ID or social security number
-
Subscriber's date of birth

Check our list of accepted insurances and also our insurance survival guide to understand how dental insurance works.
1311 Jackson Ave Dental Plan
If you do not have dental insurance, we will automatically enroll you in our in house, 1311 Jackson Ave Dental Plan. We understand that dental care should be affordable, especially for families. That is why we designed our own plan and the perks are that it is less costly than our UCR fees.
Don’t worry, you don’t have to pay anything extra for it. Other offices may charge you for that additional perk but we won’t.
Third Party Financing
If you anticipate extensive restorative work such as full smile makeovers or orthodontic treatment like Invisalign, you may want to consider CareCredit.

It is third party financing which offers flexible but extended monthly payment plans with no interest.
